Please Note: A one time initial Provider fee of $99 is required with all initial primary care, weight loss and mental health visits. This does not apply to the free one time 10 min consultation visit. All follow up Primary Care and Mental Health visits are $50. There are Care Bundles and Packages available here. Invoices will be sent prior to visit. Payments are due prior to visit and any medication refill.
